Insurance & Payment Policy

I accept most major insurance plans. During your initial appointment please bring a copy of your current insurance card and a photo ID.  As a courtesy I will file insurance on your behalf.

You are responsible for copays, co-insurance and applicable fees at the time services are rendered. I accept checks and most major credit cards. For those wishing to pay cash for services without using insurance benefits I offer competitive rates.

Cash pay clients should know that this practice abides by the “No Surprises Billing Act” disclosure, consent and good faith estimate requirements.

Cancellation Policy 

24 hours required notice for cancellation without penalty. If you cancel prior to 24 hours, then you may be subject to a $50.00 cancellation fee.
