Insurance & Payment Policy

Initial Appointment

Please bring a current insurance card and a valid photo ID to your initial appointment. As a courtesy, insurance claims will be submitted on your behalf.

Clients are responsible for copayments, coinsurance, deductibles, and any applicable fees at the time services are rendered. Payment may be made by check or most major credit cards. Competitive self-pay rates are available for clients who choose not to use their insurance benefits.

Self-pay clients should be aware that this practice complies with the requirements of the No Surprises Act, including disclosure, consent, and Good Faith Estimate provisions.

Cancellation Policy 

A minimum of 24 hours' notice is required to cancel or reschedule an appointment without penalty. Appointments canceled with less than 24 hours' notice may be subject to a $75.00 cancellation fee.
